Oral Semglutide 25mg (Wegovy Pill): What Researchers Should Know About the 2026 Formulation

For years, semaglutide was synonymous with a weekly injection. That changed in December 2025, when the FDA approved oral semaglutide 25mg, marketed as the Wegovy pill, making it the first oral GLP-1 receptor agonist cleared specifically for chronic weight management in adults. Novo Nordisk launched the product in the U.S. in early January 2026, and the scientific and research community is now taking a close look at what this formulation means beyond the clinical setting.

The Core Challenge: Oral Bioavailability of Peptides

Semaglutide is a large, complex molecule that would ordinarily be degraded by stomach acid and proteolytic enzymes before it could reach systemic circulation in meaningful concentrations. Early oral GLP-1 attempts produced modest bioavailability, enough for glycemic control at lower doses like the 7mg and 14mg Rybelsus tablets approved in 2019, but not sufficient to drive the weight loss outcomes seen with injectable Wegovy at 2.4mg weekly.

The 25mg oral formulation addressed this problem using SNAC, or salcaprozate sodium, an absorption enhancer that works by locally raising the gastric pH around the tablet. This protects semaglutide from acid degradation and facilitates its uptake through the stomach lining directly into the bloodstream. Because the drug is absorbed in the stomach rather than the gut, bioavailability is meaningful enough to achieve pharmacological effects comparable to the injectable.

The FDA approval was built on results from the Phase III OASIS 4 trial (NCT05564117), published in The New England Journal of Medicine in late 2025. They were randomized 2:1 to once-daily oral semaglutide 25mg or placebo for 64 weeks, with a 12-week dose-escalation phase included in that period.

In the treatment-policy analysis, which accounts for all participants including those who stopped treatment early, semaglutide produced a mean body weight reduction of 13.6% compared to 2.2% with placebo. When looking only at participants with full treatment adherence, that figure climbed to 16.6% mean weight loss. One in three adherent participants, specifically 34.4%, achieved at least 20% body weight reduction, compared to 2.9% on placebo. On the co-primary endpoint of achieving at least 5% weight loss, 76.3% of semaglutide participants reached that threshold versus 31.3% in the placebo group.

Post hoc analyses presented at ObesityWeek 2025 expanded the picture considerably. Researchers found meaningful improvements across a range of cardiometabolic markers including HbA1c, fasting plasma glucose, fasting serum insulin, blood pressure, LDL cholesterol, and triglycerides. Notably, 71.1% of participants who had prediabetes at baseline achieved normal blood glucose levels by week 64, compared to 33.3% with placebo. These cardiometabolic benefits were observed regardless of the degree of weight loss, though they were more pronounced in participants who achieved greater than 15% weight reduction. Participants who reported low physical function at baseline, including difficulties with bending, walking, and standing, also showed meaningful improvements in the semaglutide group.

Serious adverse events occurred in 3.9% of semaglutide participants versus 8.8% in the placebo arm.

Wegovy tablets are supplied in four strengths: 1.5mg, 4mg, 9mg, and 25mg. The titration schedule progresses through the lower doses before reaching the 25mg maintenance dose. The prescribing information includes a boxed warning regarding the risk of thyroid C-cell tumors observed in rodent studies, and the compound is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. These warnings apply equally to the injectable and oral formulations.

One drug interaction worth noting in a research context: oral semaglutide can increase the absorption of levothyroxine through its localized pH-raising effect in the stomach. This may affect TSH levels in subjects on thyroid medication, and represents a mechanistic interaction that is specific to the oral delivery route rather than the molecule itself.

At the same time, the strict fasting requirement introduces its own compliance variable that must be controlled for carefully in any protocol.

The question of body composition remains one of the most active in GLP-1 research. Studies suggest that a meaningful proportion of weight lost on semaglutide involves lean muscle mass alongside fat mass, a concern shared with tirzepatide though to a lesser extent. Whether the daily pharmacokinetic curve produced by the oral formulation yields meaningfully different body composition outcomes compared to the weekly injectable peak-and-trough profile is an open and clinically important question.

The arrival of oral semaglutide also sets the competitive and scientific context for what is coming next in the oral GLP-1 space. Eli Lilly’s orforglipron, a first-in-class non-peptide small-molecule GLP-1 receptor agonist, has been submitted to the FDA and is awaiting a regulatory decision. Unlike SNAC-based formulations, orforglipron does not involve peptide delivery at all, eliminating the gastric absorption challenge entirely and removing the fasting requirement. How these two approaches compare in real-world adherence, pharmacokinetics, and metabolic outcomes will be a defining research question in the coming years.

Competitive and Regulatory Landscape

The OASIS 4 approval gives Novo Nordisk a first-mover position in the oral GLP-1 obesity market. The approval covers chronic weight management in adults with obesity or overweight with at least one weight-related condition, as well as reduction of risk of major adverse cardiovascular events in certain adults with established cardiovascular disease.
